Please take a few minutes to answer the following questions and to tell us a little about yourselves. While this questionnaire is a little extensive, it will help us to evaluate whether one of our puppies would be a good fit for your family. Please answer all questions as completely and honestly as possible. This is our best tool for choosing a puppy that would best fit into your life!

All information is confidential, and will not be shared. It will be used as a screening tool to properly place our puppies.
